ALL CLIENTS START HERE

All Clients must complete and return the Client Agreement  and Health History before or at the FIRST APPOINTMENT ONLY. They can be emailed  or mailed.  You only need to complete these forms once.                                                            

Please take the required bloodwork list to your healthcare provider if you are obtaining labs on your own to ensure all of the necessary labs are ordered for the analysis report.
